New and Returning Student Athlete Medical Forms
Every Nichols College student must complete the medical requirements for the College through the BisonCare health portal. Student-athletes must complete additional requirements to participate in intercollegiate athletics through SportsWare Online. All paperwork for student-athletes, both new and returning, is due on August 1st, 2026. It must be submitted to SportsWare Online, and all information and documents will be reviewed by an athletic trainer before being accepted.

Please note: Per the NCAA, every student-athlete must complete a baseline concussion assessment before beginning participation in intercollegiate athletics. Nichols College partners with SWAY Medical for this. This is an additional medical clearance requirement and will take place after arrival on campus in the fall.
